North Texas Neurosurgical & Spine Center strives to provide you with timely, quality care. To help us achieve this goal, please arrive on time and keep every scheduled appointment.

Please read the following information regarding cancellations, no-shows and late arrivals.

New Patients

Before your appointment, please download and print the new patient forms, and bring the completed forms with you. If you have accessed this, please let the office know when you make the appointment. This early arrival allows our staff to efficiently perform its task of registering you into our systems.

Insurance and I.D. Cards: Please bring your insurance card and driver’s license or picture I.D. If your insurance company requires a referral for your first visit, please make sure that your referring doctor has provided this.

We will file your insurance claims on your behalf and help you with questions or concerns regarding payment of your bill. If you would like to speak with our financial counselor about questions or concerns regarding your insurance coverage or bill, please don’t hesitate to ask for a consultation.

Medications: Bring a complete, up-to-date list of all medications you take, including dosage information, or bring the bottles and packages with you.

Follow-Up Visits

For all visits, we ask that you arrive on time. Should you arrive more than 15 minutes late, your appointment may be moved to work-in status or rescheduled.


If you have an appointment and need to reschedule or cancel, please notify our office at least 24 hours prior to your appointment. We will gladly reschedule a more convenient appointment time.


In an emergency, go to the emergency room at your nearest hospital. The emergency room staff will contact us or the physician on call.
